Shared Mental Models of Provider Roles in Cancer Survivorship Care

A continuing education article for nurse practitioners, physician assistants, clinical nurse specialists, advanced degree nurses, oncology and hematology nurses, pharmacists, and physicians The purpose of this article is to provide an integrative review to analyze cancer survivor, primary care provider, and oncology provider views in survivorship care using the conceptual framework of shared mental models. The activity's target audience will consist of nurse practitioners, physician assistants, clinical nurse specialists, advanced degree nurses, oncology and hematology nurses, pharmacists, and physicians. After completing this educational activity, participants should be able to: 1. Define the concept of " shared mental models, " and list the factors that comprise such models 2. Describe how shared mental models contribute to the provision of survivorship care in cancer patients 3. Outline how oncologists, primary care providers, and cancer survivors perceive the roles of oncologists and primary care providers in cancer survivorship care 4. Describe the key barriers to the provision of optimal survivorship care 5. List the potential advantages of a written survivorship care plan 6. Identify ways in which survivorship care can be improved at their own institution.
